Then comes the pivot: a plea for help, request for advice, support with a job application, an endorsement for a competitive fellowship, or an introduction to a senior leader.</span></p><p><span>I never fault the sender. I read them all, and I know exactly what drives them: a failed exam, an upcoming interview, a fractured training pathway, and the sudden, terrifying realisation that simply passing exams is no longer enough to progress or secure the job. They are reaching out because they are out of options and a deadline is looming.</span></p><p><span>The problem is not that they are asking for help. Their problem is the timing.</span></p>

Recently, I looked at an application for a consultant job that was visually exhausting. It listed an MBBS, a BSc, an MRCP, a Master&#8217;s in Medical Education, a PG Cert in Leadership and three diplomas.</span></p><p><span>They had spent thousands of pounds and hundreds of hours accumulating institutional acronyms. When I interviewed them and asked, </span><em><span>&#8220;What is the fundamental failure in how we deliver care to this demographic, and how would you fix it?&#8221;</span></em><span> they gave me a blank stare, followed by a generic summary of a NICE guideline.</span></p>

We had no NICE guidelines, no randomised controlled trials, and no precedent. A senior clinician looked at the electronic whiteboard mapping our critical care capacity and said, </span><em><span>&#8220;We don&#8217;t have enough evidence to cancel urgent surgery yet.&#8221;</span></em><span> By the time we had the undeniable evidence, the ICU would have been completely overrun. That hesitation isn&#8217;t caution; it is paralysis masquerading as clinical rigour.</span></p>

A foundation doctor stares at a screen, realising their annual leave request for their sister&#8217;s wedding has been auto-rejected by a system algorithm.</span></p><p><span>Beside them, a medical student who is paying over nine thousand pounds a year stands in silence, waiting for a consultant to notice them so they can get a mandatory portfolio signature.</span></p><p><span>Both of them are physically present, but entirely powerless. They look at the patient list, look at the lack of staffing, and you can see the exact moment the exhaustion sets in. They have not even seen the first patient, but they are already drained. They are operating in a state of chronic, quiet surrender.</span></p>
